Our fees vary depending upon the intensity and complexity of services rendered. Please contact our billing office at 947-8551 if you have any questions about your bill.
Credit Policy:
1. We request that you pay in full at your first visit. If you have a co-pay or have already met your deductible, we ask that you be prepared to pay that at the time of service. If you do not have medical insurance, we ask that you please pay in full at each visit. We accept cash, checks, debit cards, Visa and Mastercard.
2. We will submit to insurance for you.
3. We must have a copy of your current insurance card on file in order to bill insurance.
4. Please bring your insurance card with you to your visit. We will copy it and keep it on file for you. Please also keep us informed if your insurance or any other basic information changes. We will ask that you review and sign our credit policy at your first visit.
5. We participate with Blue Cross, Blue Shield, and several other insurance plans. However, it is your responsibility to obtain any special authorizations that your individual plan might require and to know the requirements and exclusions of your individual plan. Please recognize that our service is to you, not to your insurance company. As such, you are ultimately responsible for your bill. We will do our best to work with you and your insurance company. |
