In order to provide the best possible medical care, it is helpful for you to fill out our medical information forms before your first office visit. In this section, you can also review important insurance policies, payment policies and prescription refill information.
We are committed to providing you with individualized care which is high-quality and cost effective. We look forward to being your partner in healthcare. We take appointments throughout the day on Monday through Friday. We schedule physicals, follow-ups, and care for chronic conditions so that we can devote an appropriate amount of time for you. We will also see you without an appointment on an urgent basis for injuries or acute illness. We are open Saturdays for acute visits without an appointment. We will always do our best to be prompt and see you at your appointed time. If we are running late, our staff will keep you informed about any delays. Please help us by arriving on time for your appointment. If you have come as a walk-in, we will see you as soon as we can, and we will try to keep you informed about any undue waits. We do have an obligation to see appointment patients at their appointment time, unless there is a true emergency. When you call for an appointment, our reception staff will ask a few questions so that we can schedule you in the most effective manner. We have lab, x-ray and extensive testing available on site to maximize convenience for you.
Emergency and After Hours:
After hours call our main number (947-7700). Your call will be triaged by our answering service. We utilize registered nurses to answer basic questions. If you have a complex or difficult problem, we will page the on-call physician. Neither we or technology are perfect, and we want to be available when you need us. If you have a medical emergency, please go directly to the nearest hospital Emergency Department or call 911. We are on staff at both St. Lukes Downtown and St. Alphonsus. |
