We look forward to your visit! Capital City Family Medicine is located at 1520 W. State St. (on the corner of 15th and State St.) in the Plaza 1520 building. The office is opened Monday through Friday 8:00am - 8:00pm, and Saturdays 10:00am – 4:00pm. We are closed on Sundays.
Directions to CCFM from W. Boise & Meridian:
Take I84 east to connector. Get off at River Street exit. Take a left onto 15th Street heading North. Cross over State Street and take the immediate left into our parking lot just before the laundromat.
Directions from Downtown & S.E. Boise:
Take State Street heading west. Turn right onto 15th Street. Take the immediate left into our parking lot just before the laundromat.
Directions from Eagle:
Take State Street heading east. Turn left onto 15th Street. Take the immediate left into our parking lot just before the laundromat.
Directions from Horseshoe Bend:
Take Highway 55 South to State St. Turn Left on State Street heading east. Turn left onto 15th Street. Take the immediate left into our parking lot just before the laundromat.
